Does my private insurance cover foot orthotics?

The one and only way to find out if your private insurance covers custom podiatric foot orthotics is to submit a PRE-AUTHORIZATION REQUEST (form below).

How to do it? If you would like to contact your insurer before placing your order to determine if your orthotic purchase is eligible for coverage, please complete the form below and we will send you a sample receipt. These documents can be sent to your insurer to establish eligibility and determine your coverage.

Once you submit the pre-authorization request form below, you will receive the required document within 48 hours.

Are the costs for a podiatric consultation covered by the Régie de l’assurance maladie du Québec (RAMQ)?

Except for exemptions for specialized wound care programs, podiatric consultation fees are not reimbursed by the RAMQ.

Do insurance companies cover foot orthotics?

Podiatry care and orthotics are often covered by insurance companies. However, check with your insurance provider before making an appointment with a podiatrist, as coverage varies from company to company.

Will I get a receipt after the podiatry service?

Receipts for insurance companies are provided, as is the case for office visits. (It is advisable to check the eligibility of the service with your insurance before the teleconsultation)

How do I receive the documents required to make my reimbursement request to my insurance companies?

All documentation provided by PodFormance such as your receipt for the teleconsultation or the receipt, proof of manufacture and biomechanical report for the foot orthoses will be available once your order is complete.

Once your order is complete, you will receive a physical copy of the documentation upon delivery of your order.

Additionally, the documentation will be available for download on our website. To download, please log in to your account and select the corresponding order number.

Can I buy foot orthotics and then get a prescription for foot orthotics later?

The answer is NO.


Please note that it is required to obtain a medical prescription for foot orthotics BEFORE ordering your foot orthotics for them to be covered by your insurance. Otherwise, your insurers may refuse to reimburse you for your foot orthotics since the purchase date of your orthotics is prior to the prescription date. You must always obtain the medical prescription before paying for your foot orthotics.

What documents do I need to obtain in order to submit my claim to my private insurance for reimbursement of my foot orthotics?

Although each private insurance may have its variations, here are the main documents requested by private insurance;

-A medical prescription for custom-made foot orthoses

-A biomechanical examination report

-A receipt confirming proof of purchase of the foot orthotics

- Proof of manufacture

Please note that all of these elements will be provided to you following the order of your Podform3D™ foot orthoses.
